Registration
You will need to complete and bring in each of the first three documents listed below to complete registration for a class. Please note that all three forms are required for participation.
- Application
- Waiver
- Health Clearance Form (Par-Q)
- Medical Clearance Form Only needed if applicable (see Par-Q form)
Registration Hours:
Mon.- Fri. 8:45am to 4:30pm
Please be sure to have all three forms and Panther Funds ready when you arrive.
